CONSUMER ACCESS TO HEALTH RECORDS AND COMPLAINTS

If you would like to request access to your health care records from the University of North Texas Kristin Farmer Autism Center: Email COE-AutismCenter@unt.edu. Please include name, dates of birth, and dates of attendance.
